Awards Luncheon Registration and Publication Release Form

Parents and Legal Guardians: Please fill out this form carefully to provide consent for your child’s essay and photo to be published. You will also use this form to register for the MAXCourage Children’s Celebration, our annual awards lunch for the published authors!

This year’s celebration is on Thursday, May 21, from 12:00 to 2:30 p.m., in Dorchester. In addition to themself and their teacher, each student may bring two adult guests.

  • Photography and Publication Consent, Release and Assignment

    This Parent/Guardian Consent, Release, and Assignment (“Agreement”) is entered into by the undersigned parent or legal guardian (“Parent/Guardian”) of the below-named student (“Student”) in favor of The Max Warburg Courage Curriculum, Inc., its affiliates, partners, successors, licensees, and assigns (“MAXCourage”). The Student participated in an educational writing program in which students were invited to submit personal narrative essays about a time they were called to show courage.

    Consent to Publication

    In consideration of the Student’s participation, Parent/Guardian consents and grants permission for the Student’s first and last name, essay, school identity, grade level, and photograph to be printed in The Courage of Children: Boston and Beyond, a printed and online publication of MAXCourage, and any other publication, at the sole discretion of MAXCourage, without compensation or further notice. Parent/Guardian also consents to the use of the Student’s essay, photograph, likeness, voice, video and/or identity, including composites, without compensation or further notice, in any publication by or for the benefit of MAXCourage, including but not limited to public or private displays, exhibits, broadcasts, newspapers, publications, brochures, videotapes, and electronic broadcast or publication on the World Wide Web, in such manner and at such times and in such places as MAXCourage shall determine in its discretion.

    Privacy Notice

    MAXCourage processes personal information in accordance with its Privacy Notice, which explains how personal information is collected, used, shared, and protected, as well as the rights that individuals may have regarding their personal information under applicable law.     By signing this Agreement, Parent/Guardian acknowledges that they have been directed to the Privacy Notice and understand that it describes how personal information may be handled in connection with the publication.

    Assignment of Rights

    Parent/Guardian, on behalf of the Student, hereby irrevocably assigns and transfers to MAXCourage all rights, title, and interest in and to the work, including all copyright and other intellectual property rights, in all media now known or later developed, worldwide and in perpetuity. All essays and photographs are the property of MAXCourage. Parent/Guardian hereby forever assigns all right, title, and interest in the copyright of the Student’s essay and/or photograph to MAXCourage.

    Release

    Parent/Guardian does hereby RELEASE, discharge, and covenant to hold harmless MAXCourage from any and all causes of action, liability, claims or lawsuits on account of or arising, directly or indirectly, from the Student’s participation in the MAXCourage essay program or contest, including but not limited to all claims for known or unknown injuries, all claims arising from or related to the use, publication, display and/or broadcast of the Student’s photograph, likeness, identity, voice, or essay in any form, and all claims or legal rights of action for damages which the Parent/Guardian or Student has or hereafter may acquire, either before or after he/she has reached the age of majority, resulting from his/her participation in the MAXCourage essay program or contest.

By typing my name in the signature field below and submitting this form, I certify that I am the parent or legal guardian of the student identified above and that I have the legal authority to enter into this Consent, Release, and Assignment on the Student’s behalf. I acknowledge that I have read and understand this Agreement, including the permissions, releases, and assignment of rights described herein, and I voluntarily agree to all of its terms on behalf of myself and the student. By typing my name below and submitting this form, I intend to sign this Agreement electronically and to be legally bound by its terms.
